Hospitalization Criteria for Pyelonephritis

Clinical Indicators Requiring Admission

  • Women with pyelonephritis should be hospitalized if they have severe illness, signs of sepsis, hemodynamic instability, inability to tolerate oral intake, failed outpatient treatment, complicated infections, pregnancy, immunocompromise, or significant comorbidities, as recommended by the Infectious Diseases Society of America 1, 2, 3
  • Sepsis or septic shock with hypotension, altered mental status, or organ dysfunction is an indication for hospital admission, according to the American College of Radiology 4
  • Urinary tract obstruction identified on imaging requires urgent decompression and hospital admission, as per the American College of Radiology 4
  • Renal or perinephric abscess is a complication of pyelonephritis that requires hospital admission, according to the American College of Radiology 4
  • Pyonephrosis (infected hydronephrosis) is a life-threatening condition that requires emergent drainage and hospital admission, as recommended by the American College of Radiology 4

Common Pitfalls

  • Failing to obtain imaging in patients with persistent fever after 72 hours of appropriate antibiotics may indicate obstruction, abscess, or resistant organisms, and is a pitfall to be avoided, according to Praxis Medical Insights 5
  • Missing urinary obstruction can rapidly progress to urosepsis and requires urgent urological intervention, as recommended by the American College of Radiology and Praxis Medical Insights 4, 5
