Asthma Management in Children

Dosing and Administration

  • For children 4 years and older with mild to moderate asthma exacerbations, 4-8 puffs of Asthalin (Salbutamol) MDI every 20 minutes for 3 doses, then every 1-4 hours as needed, is recommended by the American Academy of Allergy, Asthma, and Immunology 1, 2
  • MDI with valved holding chamber (spacer) is as effective as nebulized therapy when used with proper technique, as suggested by the American Academy of Allergy, Asthma, and Immunology 1
  • For severe exacerbations, consider adding ipratropium bromide (4-8 puffs every 20 minutes for up to 3 hours), according to the American Academy of Allergy, Asthma, and Immunology 2

Administration Technique and Monitoring

  • Use a valved holding chamber (spacer) for all children to improve medication delivery, as recommended by the American Academy of Allergy, Asthma, and Immunology 2
  • For children under 4 years, a face mask should be attached to the spacer, according to the American Academy of Allergy, Asthma, and Immunology 2
  • Regular follow-up is essential to assess response to therapy and adjust treatment as needed, as suggested by the American Academy of Allergy, Asthma, and Immunology 1

Asthma Control and Treatment Adjustment

  • Increasing need for Asthalin (more than twice weekly) indicates inadequate asthma control and may require adjustment of controller medications, as stated by the American College of Allergy, Asthma, and Immunology 3
  • Overreliance on rescue medication (Asthalin) without addressing underlying inflammation should be avoided, according to the American College of Allergy, Asthma, and Immunology 3
  • Not recognizing when to escalate treatment during worsening symptoms should be avoided, as recommended by the American Academy of Allergy, Asthma, and Immunology 1
