Management of Recurrent UTIs in Postmenopausal Women

Diagnosis and Initial Assessment

  • The European Association of Urology recommends confirming diagnosis of recurrent UTIs, defined as ≥3 culture-positive UTIs in one year or ≥2 UTIs in 6 months, in postmenopausal women 2, 4
  • The American Urological Association suggests obtaining urine culture and antimicrobial susceptibility testing with each symptomatic episode before initiating treatment in postmenopausal women 1, 4
  • A thorough history and physical examination should be performed to assess for complicating factors such as urinary incontinence, cystocele, high post-void residual urine, and atrophic vaginitis in postmenopausal women 2, 5
  • Extensive routine workup (e.g., cystoscopy, abdominal ultrasound) should not be performed unless specific risk factors are present in postmenopausal women 1, 6

Treatment of Acute Episodes

  • The European Urology Association recommends selecting first-line antibiotics based on local resistance patterns and previous culture results for acute UTI episodes in postmenopausal women 4
  • The American College of Obstetricians and Gynecologists suggests using nitrofurantoin 50-100 mg four times daily for 5 days as a first-line antibiotic for acute UTI episodes in postmenopausal women 2
  • Trimethoprim-sulfamethoxazole 160/800 mg twice daily for 3 days is also recommended as a first-line antibiotic for acute UTI episodes in postmenopausal women 1
  • Fosfomycin trometamol 3 g single dose is recommended as a first-line antibiotic for acute UTI episodes in postmenopausal women 1, 4

Prevention Strategies for Postmenopausal Women

  • The North American Menopause Society recommends vaginal estrogen therapy as a first-line intervention for preventing recurrent UTIs in postmenopausal women 1, 2
  • The American Urological Association suggests using lactobacillus-containing probiotics for vaginal flora regeneration in postmenopausal women 1, 2
  • Methenamine hippurate is recommended as a non-antibiotic alternative for preventing recurrent UTIs in postmenopausal women 1, 2
  • Increasing fluid intake, avoiding prolonged holding of urine, and maintaining adequate hydration are recommended lifestyle modifications for preventing recurrent UTIs in postmenopausal women 1, 2, 3

Antimicrobial Prophylaxis

  • The Infectious Diseases Society of America recommends using continuous or post-coital antimicrobial prophylaxis when non-antimicrobial interventions have failed in postmenopausal women 1, 2, 7
  • Nitrofurantoin 50 mg, trimethoprim-sulfamethoxazole 40/200 mg, and trimethoprim 100 mg are recommended as prophylactic antibiotics for postmenopausal women 2, 3, 7
  • The duration of prophylaxis typically ranges from 6-12 months in postmenopausal women 2, 7
