Valacyclovir Treatment for Oral Herpes

Episodic Treatment

  • The American Academy of Dermatology recommends valacyclovir 2 grams twice daily for 1 day (taken 12 hours apart) for optimal efficacy, which has been shown to significantly reduce the duration of episodes compared to placebo 1, 2
  • This short-course therapy has demonstrated a shorter median duration of episodes (4.0-5.0 days) compared to placebo, with treatment initiated during the prodromal phase or at the earliest sign of lesion development 1

Efficacy Considerations

  • Valacyclovir therapy can reduce the duration of symptoms, decrease pain associated with lesions, and accelerate healing time, with a high strength of evidence 1, 2
  • Valacyclovir may potentially prevent lesion development when started during prodrome, according to the Journal of Clinical Virology 2

Safety Profile

  • Valacyclovir is generally well-tolerated when used for oral herpes treatment, with common adverse events including headache, nausea, and gastrointestinal complaints, and no serious adverse events reported in studies 2, 3

Special Populations

  • For immunocompromised patients, standard dosing is typically used, but these patients should be monitored more closely, as recommended by the MMWR Recommendations and Reports 4
