Anticoagulation Management Before CAR T-Cell Therapy
Clinical Considerations
- The American Society of Clinical Oncology recommends stopping short-acting cytotoxic/anti-proliferative drugs at least 3 days before CAR T-cell infusion, and tyrosine kinase inhibitors (TKIs) and hydroxyurea at least 72 hours before infusion 1, 2, 3, 4, 5, 6, 9
- Patients with normal renal function should stop apixaban 48-72 hours before CAR T infusion, while those with renal impairment may require longer washout periods 3
- The European Society for Medical Oncology suggests that coagulation parameters should be normalized before proceeding with CAR T infusion, with frequent coagulation monitoring during the post-infusion period 3, 4, 8
- Complete blood counts with coagulation testing, comprehensive metabolic panels, and screening for active infection should be performed before CAR T-cell infusion 3, 4, 6, 8, 10
- Baseline vital signs and cardiac monitoring are also recommended before CAR T-cell infusion 10
Post-Infusion Management
- Anticoagulation should not be restarted until CRS has completely resolved and coagulation parameters have normalized, typically 7-14 days post-infusion 1, 2, 8
- Platelet count should be >50,000/µL before restarting anticoagulation 8
- Reduced-dose apixaban may be considered initially if bleeding risk remains elevated, with gradual increase to full therapeutic dose as clinical status stabilizes 3
