Switching from Aripiprazole to Quetiapine

Rationale for Cross-Tapering Strategy

  • Aripiprazole is a partial D2 agonist while quetiapine is a full D2 antagonist, requiring time for receptor adaptation during the switch 1
  • Abrupt discontinuation of aripiprazole could lead to withdrawal symptoms or symptom exacerbation, making gradual tapering essential 1
  • The gradual cross-titration approach is recommended to minimize treatment failure 1

Specific Switching Protocol

  • Start quetiapine 25mg at bedtime, and continue aripiprazole 5mg daily, to assess tolerability and maintain current dose during initial overlap 2, 3
  • Increase quetiapine to 50-100mg at bedtime, and reduce aripiprazole to 2.5mg daily, to gradually titrate quetiapine upward while simultaneously tapering aripiprazole 2, 3
  • Increase quetiapine to 150mg, and discontinue aripiprazole completely, to reach the target dose 2

Critical Monitoring Parameters

  • Monitor closely for extrapyramidal symptoms and parkinsonism, due to the risk during transition between different D2 receptor profiles 1
  • Monitor for sedation and orthostatic hypotension, as quetiapine is more sedating than aripiprazole 2, 3
  • Monitor for metabolic effects, including weight gain, as quetiapine has greater metabolic liability than aripiprazole 4
  • Monitor for withdrawal symptoms from aripiprazole, such as agitation, anxiety, insomnia, and headache 1

Special Populations

  • For elderly or frail patients, use lower starting doses of quetiapine (12.5-25mg) and slower titration, and reduce aripiprazole dose more gradually 1, 2
  • For patients with hepatic or renal impairment, reduce quetiapine doses and titrate more slowly, and consider extending the switching period to 3-4 weeks 1, 2, 3

Management of Aripiprazole‑Induced Biphasic Response (Evidence from Annals of Oncology 2018)

Dose Reduction of Aripiprazole

  • Reducing aripiprazole to 2.5 mg daily and monitoring for 1–2 weeks can alleviate early‑onset akathisia and agitation in patients who experience an initial brief improvement followed by depressive or numbness symptoms; this recommendation is based on a case series (low‑level evidence) that observed symptom resolution after dose reduction【5】.

Switching to Quetiapine

  • In a prospective study (moderate‑level evidence), patients who were cross‑tapered from aripiprazole to quetiapine demonstrated sustained mood improvement by day 7, whereas longer‑acting agents did not show such early benefit【5】.

  • The cross‑taper protocol involved:
