Pemetrexed-Carboplatin Administration Protocol for Advanced Non-Small Cell Lung Cancer and Malignant Pleural Mesothelioma

Patient Selection and Indications

  • The National Comprehensive Cancer Network recommends carboplatin-pemetrexed for patients with unresectable malignant pleural mesothelioma who cannot tolerate cisplatin 2, 3
  • The National Comprehensive Cancer Network suggests carboplatin-pemetrexed for patients with poor performance status or significant comorbidities 4, 5, 6

Dosing Protocol

  • The American College of Physicians and other medical societies support the administration of pemetrexed 500 mg/m² plus carboplatin AUC 5 intravenously on day 1 of a 21-day cycle, with mandatory folic acid and vitamin B12 supplementation starting at least 1 week before the first dose 1

Mandatory Vitamin Supplementation

  • The National Comprehensive Cancer Network and other medical organizations recommend vitamin B12 1000 μg intramuscularly starting at least 1 week before first pemetrexed dose, then every 9 weeks throughout treatment, and folic acid 0.4-1.0 mg orally daily, starting at least 1 week before first dose and continuing throughout treatment 1, 7

Treatment Duration

  • The National Comprehensive Cancer Network suggests administering 4-6 cycles for front-line therapy, and patients with stable or responding disease should take a treatment break rather than continuing maintenance therapy 1

Monitoring Requirements

  • The National Comprehensive Cancer Network recommends performance status evaluation before each cycle 2
  • The medical community suggests complete blood counts to assess nadir timing during cycle (days 8 and 15) 8

Special Populations

  • The National Comprehensive Cancer Network recommends that asymptomatic patients with low disease burden may be considered for observation before initiating systemic therapy, with chemotherapy planned at time of symptomatic or radiographic progression 2, 1

Important Contraindications and Precautions

  • The National Comprehensive Cancer Network and medical societies state that severe renal impairment is an absolute contraindication for pemetrexed 1
  • The medical community suggests that performance status 3-4 is a relative contraindication requiring alternative regimens, and best supportive care is recommended 2

Clinical Outcomes

  • The National Comprehensive Cancer Network and medical organizations report that median overall survival with carboplatin-pemetrexed is 12.7-14 months, which is comparable to cisplatin-pemetrexed 4
