Treatment Options for Nail Psoriasis

Introduction to Nail Psoriasis Treatment

  • The American Academy of Dermatology recommends topical vitamin D analogues combined with betamethasone dipropionate to reduce nail thickness, hyperkeratosis, onycholysis, and pain in patients with mild nail psoriasis 1, 2
  • The American Academy of Dermatology suggests topical tazarotene 0.1% cream can be used for nail psoriasis, with significant improvement in the Nail Psoriasis Severity Index when applied under occlusion 1, 3
  • The American Academy of Dermatology recommends high-potency topical corticosteroids, such as clobetasol 0.05% cream, for nail psoriasis when applied under occlusion 1, 4
  • The National Rheumatology Society conditionally recommends topical tacrolimus and topical ciclosporin as options for nail psoriasis 5, 6

Systemic Treatments for Moderate to Severe Nail Psoriasis

  • The National Rheumatology Society strongly recommends biologic DMARDs, including TNF inhibitors, IL-17 inhibitors, IL-12/23 inhibitors, and IL-23 inhibitors, as the most effective treatment for nail psoriasis, particularly for patients with moderate to severe disease or those with concurrent psoriatic arthritis 5, 6, 7
  • The National Rheumatology Society recommends biologic DMARDs, such as adalimumab, etanercept, infliximab, golimumab, secukinumab, ixekizumab, ustekinumab, guselkumab, and risankizumab, as first-line therapy for nail psoriasis with concurrent psoriatic arthritis 5, 6, 7

Important Clinical Considerations

  • The American Academy of Dermatology notes that topical treatments have limitations in treating severe nail disease due to poor penetration of the nail plate 1, 2
  • The American Academy of Dermatology advises against simultaneous use of salicylic acid with calcipotriene, as the acid pH will inactivate calcipotriene and reduce its effectiveness 1, 3
  • The American Academy of Dermatology warns that tazarotene is contraindicated during pregnancy and should be discontinued if pregnancy is recognized 3, 4

Nail Psoriasis Treatment

Introduction to Nail Psoriasis Management

  • The American Academy of Dermatology recommends injections of triamcinolone acetonide 5-10 mg/cc for nail matrix involvement when fewer than 3 nails are affected, with a treatment approach based on disease severity 8, 9
  • Systemic treatments become necessary when more than 3 nails are affected, or when topical treatments have failed, according to the American Academy of Dermatology and the European League Against Rheumatism 8, 9

Systemic Treatment Options

  • The American Academy of Dermatology recommends acitretine 0.2-0.4 mg/kg/day for patients with more than 3 affected nails 8
  • Methotrexate is recommended, particularly in the presence of significant skin involvement, by the American College of Rheumatology 9
  • Cyclosporine may be effective but should be limited to less than 12 consecutive months due to cumulative nephrotoxicity, as suggested by the European League Against Rheumatism 9
  • Apremilast is a second-line option for patients with nail psoriasis, according to the American Academy of Dermatology 8

Treatment of Nail Matrix Involvement

  • Injections of triamcinolone acetonide 5-10 mg/cc are more effective for nail matrix involvement, while topical treatments are more effective for nail bed involvement, as recommended by the American Academy of Dermatology 8

Nail Psoriasis Treatment Alternatives

Topical Treatment Options

  • The American Academy of Dermatology recommends topical vitamin D analogues, such as calcipotriol, as a first-line treatment for nail psoriasis, which can be applied once daily to the nail bed and surrounding tissue 10
  • The American Academy of Dermatology suggests that topical tacrolimus can be combined with calcipotriol for enhanced efficacy in treating nail psoriasis 11
  • Topical tazarotene 0.05-0.1% gel is an effective retinoid alternative for nail psoriasis, but it is teratogenic and contraindicated in pregnancy, and females of reproductive potential must use effective contraception 12

Systemic Treatment Options

  • The National Rheumatology Society recommends biologic DMARDs, such as TNF inhibitors, IL-17 inhibitors, or IL-23 inhibitors, as the most effective systemic treatment for nail psoriasis, particularly in patients with concurrent psoriatic arthritis 11
  • The American Academy of Dermatology suggests that acitretin 0.2-0.4 mg/kg daily can be used as a second-line systemic option for patients with more than 3 affected nails 13
  • Methotrexate 15 mg/week is recommended, especially with significant skin involvement, as a second-line systemic option for nail psoriasis 11

Intralesional Treatment Options

  • Intralesional triamcinolone acetonide 5-10 mg/cc is highly effective for nail matrix involvement when fewer than 3 nails are affected, and it can be tolerated even with topical steroid reactions 13

Treatment of Nail Psoriasis

Definition and Severity of Nail Psoriasis

  • Moderate-to-severe disease includes more than 3 affected nails, significant functional impairment, or concurrent psoriatic arthritis, as defined by the European League Against Rheumatism 14

Systemic Therapy for Moderate-to-Severe Nail Psoriasis

  • Biologic DMARDs, such as TNF inhibitors, are strongly recommended as the most effective treatment for moderate-to-severe nail psoriasis, particularly when concurrent psoriatic arthritis or extensive skin disease is present, according to the American College of Rheumatology 15
  • Etanercept may be less effective in patients with high BMI, as shown in phase 3 trials, with a strength of evidence rated as moderate 15

Critical Clinical Pitfalls to Avoid

  • Never use systemic corticosteroid monotherapy for psoriasis, as skin disease can flare during or after taper, potentially triggering pustular or erythrodermic forms, as warned by the National Psoriasis Foundation 14, 15

Special Considerations

  • Nail psoriasis is considered a vulnerable area that may warrant systemic therapy even with limited body surface area involvement, due to the high risk of progressive psoriatic arthritis, as noted by the European League Against Rheumatism 14, 15

Treatment of Nail Plate and Nail Bed Psoriasis

Severity-Based Treatment Algorithm

Introduction to Nail Psoriasis Treatment

  • Clinical improvement with topical treatments may not be synonymous with mycological cure when fungal superinfection is present, as noted by the British Journal of Dermatology 16

Systemic Therapy for Moderate-to-Severe Disease

Biologic DMARDs for Nail Psoriasis

  • Adalimumab demonstrated 47% mNAPSI 75 response at 26 weeks in nail psoriasis trials, with 49% achieving PGA-F clear or minimal with ≥2-grade improvement 16
  • IL-17 inhibitors may have superior short-term efficacy compared to IL-23 and TNF-alpha inhibitors, though long-term efficacy is similar to TNF-alpha inhibitors, according to the guideline society, the American Academy of Dermatology 16
