Mirabegron Dosing for Overactive Bladder

Special Considerations

  • The American Urological Association recommends that for elderly patients (≥80 years) with multiple comorbidities, the 25 mg dose of mirabegron is particularly appropriate, producing statistically significant improvements in voiding symptoms, quality of life, and bladder condition after 3 months, with an acceptably low adverse event rate of 24.62% 1, 2

Combination Therapy

  • The European Association of Urology suggests considering adding solifenacin 5 mg once daily to mirabegron at maximum dose (50 mg) if monotherapy provides inadequate response after 6 months, with the strongest evidence supporting two validated regimens: mirabegron 25 mg + solifenacin 5 mg OR mirabegron 50 mg + solifenacin 5 mg, demonstrating superior efficacy compared to either monotherapy, with effect sizes (0.65-0.95) exceeding monotherapy (0.36-0.56) 3, 1, 2

Monitoring Requirements

  • The American College of Cardiology recommends monitoring blood pressure periodically, especially during initial treatment with mirabegron, as it can cause dose-dependent blood pressure increases, and is contraindicated in severe uncontrolled hypertension 2
