Alprazolam Dosing Considerations

Special Population Considerations

  • The American Geriatrics Society recommends a starting dose of 0.25 mg given 2-3 times daily for elderly and debilitated patients, with a maximum dose of 2 mg in 24 hours 1, 2
  • The National Comprehensive Cancer Network suggests lower starting doses and slower titration for elderly and debilitated patients 3
  • For patients with hepatic impairment, the starting dose should be 0.25 mg given 2-3 times daily, with cautious dose adjustments 1, 2

Drug Interactions Affecting Dosing

  • The American Academy of Family Physicians recommends reducing the alprazolam dose by 50% when coadministered with nefazodone, and exercising caution when using fluvoxamine with alprazolam 4, 5
