Haloperidol IV Dosing for Abdominal Pain Management

FDA-Approved Dosing Parameters

  • The FDA recommends an initial dose of 2-5 mg IM/IV for prompt control of acute symptoms, with a maximum daily dose of 20 mg per day, and debilitated or geriatric patients require lower doses with more gradual titration 1

Safety Monitoring Requirements

  • Before administering haloperidol IV for abdominal pain, it is essential to check the QTc interval and avoid administration if prolonged, as haloperidol prolongs QTc at steady-state, and have diphenhydramine or benztropine immediately available for acute dystonic reactions 1

Common Pitfalls to Avoid

  • The American College of Emergency Physicians suggests not confusing agitation dosing with analgesia dosing, as agitation protocols use combination therapy with benzodiazepines, which is unnecessary for abdominal pain 2, 1
