Management of Renal Ectasia
Evaluación inicial
- The American College of Nephrology recommends a complete evaluation including a focused medical history and physical examination to assess volume status and signs of edema in patients with renal ectasia 1
- The initial evaluation should include blood biochemistry tests, such as a complete blood count, sodium, chloride, albumin, magnesium, creatinine, urea, proteins, cholesterol, triglycerides, and glucose, to assess the patient's overall health and renal function 1
- An abdominal and pleural ultrasound should be performed to evaluate renal echogenicity and size, ascites, effusions, and thrombosis 1
Plan de tratamiento
Manejo de fluidos
- The European Renal Association recommends fluid restriction in cases of hyponatremia and severe edema, and limiting fluid intake when possible, using concentrated formulas with high caloric content to meet energy needs related to age 1
- The assessment of volume status and implementation of salt restriction should be performed to manage fluid balance in patients with renal ectasia 1
Tratamiento farmacológico
- The Kidney Disease Improving Global Outcomes (KDIGO) guidelines recommend initiating furosemide at a dose of 0.5-2 mg/kg per oral dose up to six times a day (maximum 10 mg/kg per day) according to the degree of edema and diuresis achieved 2
- A thiazide diuretic may be considered to be added to achieve a synergistic effect with loop diuretics in stable patients 2
- Albumin infusions should be considered only if there are clinical indicators of hypovolemia (oliguria, prolonged capillary refill time, tachycardia, hypotension) 2
Seguimiento y monitorización
- The International Society of Nephrology recommends that children with established therapy and stable condition should be seen at least every 6-12 months, and adults every 6-12 months 3
- Renal ultrasound should be performed at least every 12-24 months to monitor nephrocalcinosis, appearance of kidney stones, and signs of secondary obstructive uropathy 3
- Quality of life should be evaluated using appropriate age-related scales every 2 years from the age of 5 3
Seguimiento específico en niños
- At each follow-up visit, focus on history and examination of dehydration, degree of polyuria, signs of muscle weakness, growth, and psychomotor development 3
- Biochemical workup should include acid-base status, serum electrolytes (including bicarbonate, chloride, and magnesium), renal function, PTH, and urinary calcium excretion 3
- Urine osmolality should be evaluated to detect nephrogenic diabetes insipidus 3
Seguimiento específico en adultos
- At each follow-up visit, focus on history and examination of dehydration, degree of polyuria, signs of muscle weakness, fatigue, and palpitations 3
- Biochemical workup should include acid-base status, serum electrolytes, renal function, PTH, urinary calcium excretion, and microalbuminuria 3
- Additional cardiac workup should be performed in patients complaining of palpitations or syncope 3
Complicaciones y manejo
- The National Kidney Foundation recommends monitoring renal function to detect early signs of chronic kidney disease 4
