Antifungal Selection to Avoid Interactions with Plaquenil (Hydroxychloroquine)

Primary Recommendations by Drug Class

  • The standard dosing for micafungin is 100 mg daily, anidulafungin is 200 mg loading then 100 mg daily, and caspofungin is 70 mg loading then 50 mg daily, as recommended by the Journal of Microbiology, Immunology and Infection 1, 2

Clinical Algorithm for Selection

  • For invasive candidiasis, the Infectious Diseases Society of America recommends using echinocandins (micafungin or anidulafungin preferred) as first-line therapy, with amphotericin B formulations as acceptable alternatives 3, 4
  • For invasive aspergillosis, the Infectious Diseases Society of America recommends using liposomal amphotericin B or echinocandins 5, 4
  • For cryptococcosis, the Infectious Diseases Society of America recommends using amphotericin B deoxycholate plus flucytosine as induction therapy, and avoiding fluconazole consolidation therapy if possible 2, 4
  • For mucormycosis, the Infectious Diseases Society of America recommends using liposomal amphotericin B as the drug of choice, with no interaction concerns 1, 4
