Cluster Headache Prevention and Treatment

First-Line Preventive Treatments

  • The American College of Cardiology and other guideline societies recommend galcanezumab as first-line prophylactic treatment for episodic cluster headache with the strongest evidence among available options 1, 2
  • The VA/DoD guidelines provide a weak recommendation for galcanezumab specifically for episodic (not chronic) cluster headache 3, 1
  • Verapamil (at least 240 mg daily, with maximum dose depending on efficacy and tolerability) is commonly used, though recent VA/DoD guidelines note insufficient evidence to recommend for or against it 3

Chronic Cluster Headache Prevention

  • The VA/DoD guidelines specifically do not recommend galcanezumab for chronic cluster headache prevention (weak recommendation against) 3, 1
  • There is insufficient evidence to recommend for or against verapamil for chronic cluster headache prevention 3, 1

Acute Treatment Options

  • Subcutaneous sumatriptan (6 mg) or intranasal zolmitriptan (10 mg) are recommended for acute treatment of cluster headache attacks 3, 1
  • Normobaric oxygen therapy (100% oxygen at flow rates of at least 12 L/min for 15 minutes) is recommended for acute treatment 3, 1, 2
  • Noninvasive vagus nerve stimulation is suggested for the short-term treatment of episodic cluster headache 3, 1

Important Considerations and Monitoring

  • Implantable sphenopalatine ganglion stimulator is not recommended for cluster headache treatment (weak recommendation against) 3, 1

Verapamil Dosing for Cluster Headache

Important Considerations

  • Verapamil should not be given to patients with impaired ventricular function, heart failure, or wide-complex tachycardias, as recommended by the American Heart Association, based on evidence from Circulation 4

Cardiac Monitoring Requirements

  • The American College of Cardiology recommends obtaining baseline ECG before initiating therapy and monitoring PR interval with ECG when using doses >360 mg daily, as doses used in cluster headache may be double those used in cardiology, although no specific citation is provided in this context, the general principle is supported by various cardiology guidelines, however, a relevant citation is not available in the given text, but the concept is crucial for patient safety 4

Verapamil Treatment for Cluster Headache

Contraindications and Precautions

  • The American Heart Association recommends avoiding verapamil in patients with impaired ventricular function or heart failure, AV block greater than first degree or SA node dysfunction (without pacemaker), decompensated systolic heart failure or severe LV dysfunction, and hypotension 5
  • The American Heart Association also advises caution when using verapamil due to its potential to inhibit CYP3A4 and P-glycoprotein, which can lead to significant drug interactions, including contraindication with dofetilide and requiring careful monitoring with dabigatran, edoxaban, rivaroxaban, flecainide, simvastatin, digoxin, and many other medications 5

Drug Interactions

  • The American Heart Association warns that verapamil is a moderate CYP3A4 inhibitor and P-glycoprotein inhibitor, and its use requires careful monitoring of potential drug interactions, including avoidance of grapefruit juice 5
