Osteoporosis Treatment Guidelines
Introduction to Osteoporosis Management
- The American College of Physicians recommends bifosfonat (alendronate, risedronate, zoledronic acid, or denosumab) as the primary pharmacological therapy to reduce the risk of vertebral and hip fractures in women with osteoporosis 1.
- There is no clinical evidence to support the use of fitoestrogen as an additional therapy to bifosfonat, and patients should continue bifosfonat as the primary therapy for osteoporosis 1, 2.
Bifosfonat Therapy
- The American College of Physicians recommends treatment with bifosfonat for 5 years as the standard duration for women with osteoporosis 1, 3.
- Alendronate, risedronate, and zoledronic acid reduce the relative risk of new vertebral, non-vertebral, and hip fractures compared to placebo 1.
- Patients should continue bifosfonat (alendronate, risedronate, or zoledronic acid) as the primary therapy for osteoporosis, and treatment should be continued for 5 years, with reassessment of fracture risk after this period 1, 2, 3.
Supplementation Considerations
- Ensure adequate calcium intake (800-1000 mg/day) and vitamin D (800 IU/day) during bifosfonat treatment 5, 3.
- Vitamin D deficiency should be corrected before starting bifosfonat, especially for intravenous therapy, as deficiency can reduce efficacy and increase the risk of bifosfonat-related hypocalcemia 3.
Risks and Considerations
- Osteonecrosis of the jaw occurs in 0.01-0.3% of users, with increased risk with longer duration 4, 3.
- Atypical subtrochanteric and diaphyseal femoral fractures have an incidence of 3.0 to 9.8 cases per 100,000 patient-years 3.
- Dental work should be completed before starting or continuing bifosfonat therapy to reduce the risk of osteonecrosis of the jaw 4, 3.
Alternative Therapies
- If patients experience intolerable side effects or inadequate response to bifosfonat, consider switching to zoledronic acid intravena, denosumab, or teriparatide 7.
- Denosumab is recommended as a second-line therapy when bifosfonat is contraindicated or not tolerated 7.
- Denosumab should not be discontinued without immediately starting bifosfonat therapy (within 6 months), as vertebral fracture rebound can occur 7, 3.
