Calculating Prefilter D5W Rate for Hyponatremic Patient on CRRT

Rationale for Prefilter D5W in Hyponatremia

  • Rapid correction of hyponatremia can lead to osmotic demyelination syndrome (ODS), a serious neurologic complication, characterized by signs such as dysarthria, dysphagia, oculomotor dysfunction, and quadriparesis, with risk factors including advanced liver disease, alcoholism, severe hyponatremia, malnutrition, and metabolic derangements 1

Calculation Method

  • For patients with chronic hyponatremia, the goal rate of increase should be 4-6 mEq/L per 24-hour period, not exceeding 8 mEq/L per 24-hour period, according to the American Association for the Study of Liver Diseases 1

Implementation Guidelines

  • Monitor serum sodium every 4-6 hours to ensure correction rate does not exceed 8 mEq/L in 24 hours, as recommended by the American Association for the Study of Liver Diseases 1

Special Considerations

  • Warming the dialysate helps maintain hemodynamic stability during CRRT, as suggested by the European Renal Association 2

Potential Complications and Monitoring

  • Watch for signs of ODS and monitor for other electrolyte abnormalities that may develop during CRRT, with risk factors for ODS including advanced liver disease, alcoholism, severe hyponatremia, malnutrition, and metabolic derangements 1

Management of Hyponatremia in CRRT Patients

Rationale for D5W Administration

  • Pre-filter D5W administration enhances the achievable ultrafiltration rate, which is especially important in high-volume CVVH, and can be useful in patients with frequent filter clotting, as recommended by the Kidney International guidelines 3, 4, 5

Potential Complications and Monitoring

  • Monitor for other electrolyte abnormalities that commonly develop during CRRT, including hypophosphatemia, hypokalemia, and hypomagnesemia, according to Clinical Nutrition guidelines 6
  • Ensure proper fluid balance monitoring, as adaptive use of intravenous infusion pumps for CRRT has been shown to risk significant errors in fluid balance, as reported in Kidney International 3

Calculating Prefilter D5W Rate for Hyponatremic CRRT Patient

Implementation Guidelines

  • The Kidney International recommends maintaining a CRRT effluent volume of at least 20-25 mL/kg/hr 7

Practical Tips

  • According to Nature Reviews Nephrology, consider warming the dialysate to maintain hemodynamic stability during CRRT 8
