Combining Amphetamine-Based Stimulants

Introduction to Stimulant Combination Risks

  • The American Academy of Child and Adolescent Psychiatry recommends avoiding the combination of Adderall and Vyvanse 50mg due to excessive total amphetamine exposure and increased risk of cardiovascular and psychiatric adverse effects 1, 2

Dosing and Titration Guidelines

  • The maximum recommended daily amphetamine dose should not be exceeded, with guidelines specifying 40-50mg total daily dose for amphetamines in adults 1, 2, 3
  • Vyvanse can be titrated upward in 10-20mg increments weekly if tolerated, with a maximum approved dose of 70mg daily for adults with ADHD 1

Alternative Treatment Approaches

  • If Vyvanse 50mg is inadequate, consider optimizing the dose by titrating upward or switching to a different stimulant formulation, such as Adderall XR or methylphenidate-based stimulants 1, 2, 5
  • If stimulants are ineffective or poorly tolerated, consider adding or switching to non-stimulant options, such as atomoxetine, guanfacine, or bupropion 1, 2, 5

Monitoring and Safety Precautions

  • Regular monitoring of blood pressure, pulse, and weight is necessary, as well as systematic assessment of therapeutic response and side effects 1, 2, 3, 4
  • Cardiovascular symptoms, such as chest pain, palpitations, or syncope, would contraindicate further stimulant use 1
