Managing Drug Interactions Between Clozapine and Fluvoxamine

Immediate Management Recommendations

  • The American Academy of Child and Adolescent Psychiatry recommends obtaining baseline clozapine blood level measurement immediately to assess current exposure in patients taking clozapine and fluvoxamine 1, 2
  • Weekly complete blood count monitoring should be implemented due to increased risk of agranulocytosis with this combination 1, 2
  • Consider either reducing clozapine dose by 50-75% while maintaining fluvoxamine, or discontinuing fluvoxamine and gradually increasing clozapine to therapeutic levels with standard monitoring, as recommended by The Lancet Psychiatry 3

Monitoring Parameters

  • The American Academy of Child and Adolescent Psychiatry recommends weekly blood monitoring for the first 6 months of clozapine treatment, then every 2 weeks thereafter 1
  • Immediate discontinuation of clozapine is recommended if WBC drops below 2,000/mm³ or absolute neutrophil count drops below 1,000/mm³, as per the guidelines of the American Academy of Child and Adolescent Psychiatry 1
  • Monitor for signs of serotonin syndrome due to the combination of two serotonergic medications, as advised by the Journal of the American Academy of Child and Adolescent Psychiatry 4

Long-term Considerations

  • The Lancet Psychiatry suggests maintaining lower clozapine doses with regular blood level monitoring targeting 350-550 ng/mL if continuing this combination 3
  • Consider metformin to mitigate potential weight gain and metabolic effects of clozapine, as recommended by The Lancet Psychiatry 3
  • Ensure comprehensive psychosocial interventions are incorporated into the treatment plan, as advised by the Journal of the American Academy of Child and Adolescent Psychiatry 2, 5
