Management of Urethral Injury

Initial Diagnostic Approach

  • The American Urological Association recommends performing retrograde urethrography immediately when urethral injury is suspected based on symptoms such as blood at the meatus, inability to void, perineal/genital ecchymosis, or high-riding prostate on exam, with a diagnostic accuracy of 95.9% 3

Management Algorithm by Injury Type

  • The World Health Organization suggests that initial conservative management is the treatment of choice for blunt trauma (straddle injuries) with urinary drainage via either urethral or suprapubic catheter, with attempt of endoscopic realignment before considering surgery 1, 2
  • The European Association of Urology recommends immediate direct surgical repair for penetrating trauma if the patient is hemodynamically stable and an experienced surgeon is available, with selected cases of incomplete penetrating injuries managed with trans-urethral catheter placement 1, 2

Urinary Drainage Strategy

  • The American College of Surgeons advises establishing urinary drainage as soon as possible in any traumatic urethral injury, with either urethral or suprapubic catheter placement acceptable for initial drainage 1, 2

Follow-Up Protocol

  • The World Health Organization suggests that ureteroscopy or urethrogram are the methods of choice for follow-up of urethral injuries, with follow-up imaging performed every two weeks until complete healing in blunt anterior injuries 1, 2

Critical Pitfalls to Avoid

  • The American Urological Association warns against attempting immediate urethroplasty for posterior urethral injuries due to high complication rates (erectile dysfunction, incontinence), and against delaying urinary drainage establishment in any urethral injury 1, 3
