Calcium and Vitamin D Supplementation for Patients on Prolia

Introduction to Calcium and Vitamin D Supplementation

  • The American Urological Association guideline notes that denosumab causes more significant hypocalcemia than other bone-protective agents, and supplemental calcium with serum calcium monitoring is mandatory for patients on Prolia 4, 5
  • For patients on Prolia, the evidence-based optimal supplementation is: Calcium 1200 mg daily and Vitamin D 800-1000 IU daily 1, 2, 3
  • Vitamin D doses of 700-800 IU/day reduce hip fractures by 26% and non-vertebral fractures by 23%, while 400 IU/day shows no benefit 4, 5
  • High-dose vitamin D (≥800 IU/day) reduces hip fracture risk by 30% in adults 65+ years 1
  • Combined calcium (1200 mg) and vitamin D (800 IU) supplementation reduces hip fracture risk by 16% 1

Practical Implementation

  • Divide calcium into doses of ≤600 mg for optimal absorption 1, 2, 3
  • Calcium carbonate (40% elemental calcium) should be taken with meals for absorption 3
  • Calcium citrate (21% elemental calcium) is preferred for patients on proton pump inhibitors and can be taken without food 2, 3
  • Standard maintenance vitamin D dose is 800-1000 IU daily 1, 2
  • Target serum 25(OH)D level is ≥30 ng/mL (75 nmol/L) 1, 2

Monitoring and Safety

  • Serum calcium should be monitored regularly, especially in the first months of treatment 4, 5
  • 25(OH)D levels should be checked after 3 months of supplementation, then every 1-2 years 1, 2
  • In high-risk patients (CKD, history of kidney stones), consider 24-hour urinary calcium 1

Special Considerations

  • Patients on glucocorticoids are already at high fracture risk, and calcium and vitamin D supplementation is especially critical 4, 5, 2
  • Patients with chronic kidney disease may need higher vitamin D doses to maintain target levels 2
