Staphylococcus aureus Bacteremia Management

Antibiotic Selection and Treatment Duration

  • The Infectious Diseases Society of America recommends starting vancomycin 15-20 mg/kg IV every 8-12 hours or daptomycin 6 mg/kg IV once daily as empirical therapy for Staphylococcus aureus bacteremia, as both cover MRSA while awaiting culture results 1, 2
  • Beta-lactams are superior to vancomycin for methicillin-susceptible S. aureus (MSSA) and are associated with lower recurrence rates, according to the Praxis Medical Insights guideline 3
  • Vancomycin with target trough levels of 15-20 mcg/mL is recommended for serious MRSA infections, and daptomycin 6 mg/kg IV once daily is equally effective, with some experts recommending 8-10 mg/kg for complicated cases 1, 2, 3

Treatment Duration

  • The Infectious Diseases Society of America recommends treating uncomplicated bacteremia for at least 14 days to prevent relapse, as shorter courses are associated with significantly higher relapse rates 1
  • Uncomplicated bacteremia requires negative follow-up blood cultures, defervescence within 72 hours, no evidence of endocarditis, no implanted prostheses, and no metastatic sites of infection, according to the Clinical Infectious Diseases guideline 1
  • Complicated bacteremia should be treated for 4-6 weeks if any criterion for uncomplicated bacteremia is not met, with a 90-day mortality risk of 39% for prolonged bacteremia, as stated in the Clinical Infectious Diseases and Praxis Medical Insights guidelines 1, 2

Critical Management Steps

  • The Praxis Medical Insights guideline recommends removing all infected intravascular catheters and implanted devices immediately, and draining abscesses and performing surgical debridement of infected tissue to prevent treatment failure 2, 3
  • Follow-up blood cultures should be obtained 2-4 days after initial positive cultures and as needed thereafter to document clearance, according to the Clinical Infectious Diseases and Praxis Medical Insights guidelines 1, 2
  • Transthoracic echocardiography should be performed on all patients with S. aureus bacteremia, and transesophageal echocardiography is preferred for patients with persistent bacteremia, fever, or metastatic infection, as recommended by the Clinical Infectious Diseases guideline 1

Special Populations

  • The Clinical Infectious Diseases and Praxis Medical Insights guidelines recommend vancomycin IV for children with MRSA bacteremia, and clindamycin 10-13 mg/kg IV every 6-8 hours can be used if clindamycin resistance is <10% and the patient is stable without ongoing bacteremia 1, 2
  • Linezolid dosing is recommended as 600 mg PO/IV twice daily for children >12 years, and 10 mg/kg every 8 hours for children <12 years, according to the Clinical Infectious Diseases and Praxis Medical Insights guidelines 1, 2
