Tofacitinib Dosing for Vitiligo

Standard Dosing Regimen

  • The American Academy of Dermatology recommends a dose of 5 mg orally twice daily for vitiligo, based on extrapolation from psoriasis dosing guidelines and clinical evidence in vitiligo patients 1
  • A 5 mg twice daily dose provides a favorable risk-benefit profile compared to the 10 mg twice daily dose, which carries significantly higher risks of serious infections, cytopenia, and pulmonary embolism 1, 2

Dose Modifications

  • For patients with moderate to severe renal or hepatic impairment, the recommended dose is 5 mg once daily 1, 3
  • Tofacitinib should be avoided entirely in severe hepatic impairment 1
  • When taking potent CYP3A4 inhibitors or combinations causing moderate CYP3A4 and potent CYP2C19 inhibition, the dose should be reduced to 5 mg once daily 1, 3

Mandatory Pre-Treatment Screening

  • Before initiating tofacitinib, a complete blood count with differential, comprehensive metabolic panel, lipid profile, tuberculosis screening, hepatitis B and C screening, and pregnancy test if applicable are required 1, 2, 3
  • Tofacitinib should not be initiated if the lymphocyte count is <500 cells/mm³, absolute neutrophil count is <1000 cells/mm³, hemoglobin is <9 g/dL, or an active serious infection is present 1, 3

Ongoing Monitoring Requirements

  • Laboratory monitoring schedule includes CBC with differential, comprehensive metabolic panel, and lipid profile at 4-8 weeks, then every 3 months, and annually for lipid profile 3
  • Tofacitinib should be discontinued immediately if the lymphocyte count is <500 cells/mm³, ANC is <500 cells/mm³, or hemoglobin decreases ≥2 g/dL or falls <8.0 g/dL 1, 3

Critical Safety Warnings

  • The American Academy of Dermatology warns that tofacitinib is contraindicated in active serious infection, and should not be combined with other potent immunosuppressants or biologics 1, 3
  • Live vaccines should be avoided during treatment 1, 3
  • High-risk populations requiring extreme caution include those with a history of malignancy, age >50 years with cardiovascular risk factors, history of recurrent infections, and patients at risk for herpes zoster 2, 3
  • Pregnancy considerations include using tofacitinib only if potential benefit justifies fetal risk, and generally advising against its use in pregnancy and lactation 1, 3

Common Pitfalls to Avoid

  • The American Academy of Dermatology advises against using tofacitinib without light exposure, as monotherapy without phototherapy or sunlight exposure is ineffective for repigmentation 2, 3
  • Escalating to 10 mg twice daily for vitiligo is not recommended, as the 5 mg dose is effective when combined with light, and higher doses substantially increase serious adverse events 2, 3
  • Baseline screening should not be skipped, as serious infections including tuberculosis reactivation can occur 2, 3
  • Combining tofacitinib with other systemic immunosuppressants increases infection and malignancy risk 1, 3
