Bile Acid Therapy Guidelines
Mechanism of Action and Clinical Evidence
- The American Association for the Study of Liver Diseases recommends UDCA at 13-15 mg/kg/day as the established first-line treatment for primary biliary cirrhosis (PBC) with proven efficacy in reducing transplantation and death 1, 2
- UDCA works primarily through stimulation of impaired hepatocellular secretion via posttranscriptional mechanisms, and changes bile acid composition from hydrophobic to more hydrophilic, reducing toxic bile acid exposure 2
- UDCA provides antioxidant, immunomodulatory and anti-apoptotic properties 3, 4
- NorUDCA has demonstrated anti-cholestatic, anti-inflammatory, and anti-fibrotic properties in preclinical models that appear superior to UDCA 3, 4
Clinical Applications and Regulatory Status
- The European Association for the Study of the Liver recommends UDCA for intrahepatic cholestasis of pregnancy at 10-15 mg/kg/day to reduce preterm birth and potentially stillbirth when bile acids are elevated 5, 1, 2
- UDCA has demonstrated biochemical efficacy (ALT reduction) in certain liver conditions, but no proof of histological efficacy 3, 4
- NorUDCA remains investigational with no large phase III trials completed for any indication, and no published histological fibrosis endpoint data exists for NorUDCA in human trials 6
- The American College of Gastroenterology notes that NorUDCA has shown promising results in primary sclerosing cholangitis (PSC) in early clinical trials, and improved liver injury in mouse models of alpha-1 antitrypsin deficiency 3, 4, 7
Nor-Ursodeoxycholic Acid for Primary Sclerosing Cholangitis
Primary Sclerosing Cholangitis Treatment
- The British Society of Gastroenterology strongly recommends against using standard UDCA for routine PSC treatment, and high-dose UDCA (28-30 mg/kg/day) is associated with increased serious adverse events, death, liver transplantation, and varices 8, 9
Standard UDCA Contraindication
- Standard UDCA is contraindicated for routine PSC treatment, making norUDCA a particularly important investigational agent for this population 8
