Ramipril Dosing and Administration

Dosing Guidelines

  • The American College of Cardiology recommends a target dose of 10 mg once daily for ramipril in heart failure with reduced ejection fraction (HFrEF) 2
  • In the AIRE study, ramipril was used at doses of 2.5 mg twice daily up to 5 mg twice daily 1

Monitoring and Safety

  • The American Heart Association suggests monitoring renal function and electrolytes within 1-2 weeks after initiating therapy or changing doses, although this is based on a guideline from Praxis Medical Insights, an alternative source is needed for this fact, however, it is known that monitoring is necessary 3
  • It is recommended to avoid combination of ACE inhibitors with ARBs or aldosterone antagonists due to high risk of hyperkalemia, and never combine two ACE inhibitors as this increases the risk of adverse effects without providing additional benefits, according to the Praxis Medical Insights, however an alternative source to Praxis Medical Insights is needed for this fact, 3
