Cross-Titration from Olanzapine to Quetiapine

Rationale and Protocol

  • The American Academy of Family Physicians recommends starting quetiapine at 25mg twice daily while maintaining olanzapine 10mg daily, then tapering olanzapine by 2.5-5mg every 3-7 days while simultaneously increasing quetiapine to a target dose of 300-400mg/day over 1-2 weeks, to minimize risk of symptom exacerbation 1, 2, 3
  • The cross-titration strategy is preferred over abrupt switching due to the patient's complex symptom profile, including anxiety, agitation, bipolar mood, and sleep disturbances, as it allows for a more gradual transition and reduces the risk of rebound symptoms 1, 2, 3

Monitoring Parameters

  • Orthostatic vital signs should be checked before each dose adjustment, as quetiapine causes more sedation and transient orthostasis than olanzapine, with a particular focus on monitoring blood pressure sitting and standing, especially during the first 2 weeks 1, 3, 5
  • Sleep quality is expected to improve as quetiapine dose increases due to its sedating properties, and should be monitored regularly 1, 5
  • Metabolic parameters, including weight, glucose, and lipids, should be monitored at baseline and follow-up, as both olanzapine and quetiapine can cause metabolic effects, although olanzapine has a higher risk for weight gain and dyslipidemia 1, 3

Dosing Considerations

  • Quetiapine is significantly more sedating than olanzapine, and a dosing strategy that gives a larger portion of the quetiapine dose at bedtime can help leverage sedation for sleep while minimizing daytime somnolence 1, 3, 5
  • The final quetiapine dose should typically be 300-400mg/day for bipolar disorder and agitation, although the effective range is 150-750mg/day, and asymmetric dosing with the larger portion at bedtime may be considered for patients with sleep disturbances 1, 3, 4

Special Populations

  • For elderly or frail patients, quetiapine should be started at 12.5mg twice daily instead of 25mg twice daily, and olanzapine should be reduced more gradually, with a focus on minimizing the risk of adverse effects 1, 3
  • In patients with hepatic impairment, lower starting doses of quetiapine and slower titration are recommended, with a focus on minimizing the risk of adverse effects and ensuring safe use 1, 3
