Anticoagulation for Atrial Fibrillation in Elderly Patients with Valve Disease

Introduction to Anticoagulation Therapy

  • The European Society of Cardiology recommends oral anticoagulation with either a vitamin K antagonist or a direct oral anticoagulant for patients with a CHA₂DS₂-VASc score ≥2, which is a Class I, Level A recommendation 1

Stroke Risk Assessment and Management

  • An elderly female with atrial fibrillation and valve disease has multiple high-risk features, including age ≥75 years, which is a moderate-risk factor with a relative risk of 1.4 per decade 2
  • Valve disease is a moderate-to-high risk factor depending on severity, according to the American College of Cardiology 4
  • The American Heart Association states that female sex with other risk factors contributes to the CHA₂DS₂-VASc score 1

Anticoagulation Therapy Options

  • The European Society of Cardiology states that apixaban is appropriate for native valve disease, bioprosthetic valves, or previous valve repair, but contraindicated for mechanical prosthetic heart valves or clinically significant mitral stenosis 1
  • Aspirin provides inadequate stroke protection in high-risk patients, reducing stroke risk by only 19% (95% CI 2-34%) in atrial fibrillation patients, according to the American College of Cardiology 4
  • Oral anticoagulation reduces stroke risk by approximately 64-68% compared to aspirin or no treatment, as reported in the Annals of Internal Medicine 6

Apixaban Dosing and Monitoring

  • The standard dose of apixaban is 5 mg twice daily unless the patient meets ≥2 of the following dose-reduction criteria: age ≥80 years, body weight ≤60 kg, or serum creatinine ≥1.5 mg/dL, according to the European Heart Journal 1
  • If ≥2 criteria are met, the dose should be reduced to 2.5 mg twice daily, as recommended by the European Society of Cardiology 1
  • Baseline creatinine clearance is mandatory before initiating apixaban, and ongoing monitoring is required, with annual CrCl assessment in patients with normal renal function and more frequent assessments in patients with moderate renal impairment, as stated in the European Heart Journal 1

Bleeding Risk Management

  • Elderly patients (≥75 years) have approximately twice the risk of serious bleeding during anticoagulation compared to younger patients, according to the American Heart Association 5
  • Addressing modifiable bleeding risk factors, such as discontinuing aspirin, controlling hypertension, avoiding NSAIDs, and reviewing all medications, is crucial before initiating apixaban, as recommended by the American College of Cardiology 4
