Maximum Recommended Dosing and Safety Guidelines for Bupropion
Maximum Dosing Limits
- The overall upper limit for bupropion (Wellbutrin) is 450 mg per day; doses above this markedly increase seizure risk. 1, 2, 3
- Wellbutrin SR: recommended maximum 400 mg/day (200 mg twice daily); some guidelines allow 450 mg/day as an absolute ceiling. 1, 2, 3
- Wellbutrin XL: recommended maximum 450 mg/day; the usual regimen is 300 mg once daily, with 450 mg as the ceiling. 1, 2, 3
- For smoking‑cessation therapy, the maximum dose is 300 mg/day. 4, 5
- For obesity treatment (bupropion combined with naltrexone), the maximum bupropion dose is 360 mg/day. 6, 7, 8
Special Populations and Dose Adjustments
- In moderate to severe renal impairment, the bupropion dose should be reduced to 150 mg once daily. 7
- In moderate to severe hepatic impairment, the dose should be reduced to 150 mg once daily or every other day. 7
- For elderly patients, initiation should be at ≈50 % of the adult dose. 10
Clinical Management Recommendations
- When a patient does not respond to the maximum approved dose (450 mg/day), clinicians should switch to a different antidepressant class rather than exceed the dose limit. 9, 10
- Prior to dose escalation, clinicians should re‑evaluate the diagnosis and ensure an adequate therapeutic trial of 6–8 weeks has been completed. 9, 10
