Switching from Valproate (Depakote) to Lithium in Bipolar Disorder – Guideline Recommendations

Baseline Laboratory Assessment

  • The American Academy of Child and Adolescent Psychiatry recommends obtaining a complete blood count, thyroid function tests (TSH and free T4), urinalysis, blood urea nitrogen and creatinine, serum calcium, and a pregnancy test (if applicable) before initiating lithium therapy. 2

Initiation Dosing of Lithium

  • Begin lithium at 300 mg twice daily; if the patient is clinically stable and not elderly, a higher starting dose of 450 mg twice daily may be used. [1][2]

Titration and Depakote Tapering Strategy

  • Gradual tapering of prophylactic mood stabilizers is advised to prevent relapse; the guideline specifies reducing the concomitant valproate dose by approximately 25 % every two weeks during the switch. [1][2]3
  • Target therapeutic lithium serum concentration should be 0.8–1.2 mEq/L, with dose adjustments guided by serum levels and tolerability. [1][2]

Clinical and Laboratory Monitoring During Transition

  • Clinical monitoring: assess patients weekly for early signs of mood‑episode relapse throughout the conversion period. [1][2]3
  • Laboratory monitoring: after a stable lithium dose is achieved, re‑check lithium levels, renal function, and thyroid function every 3–6 months. [1][2]3

Safety Considerations

  • Avoid abrupt discontinuation of valproate: sudden cessation carries a high risk of seizure recurrence (if the drug is used for epilepsy) and mood‑episode relapse; a gradual taper is required. [1][2]3
  • Maintain aripiprazole (Abilify) unchanged (e.g., 30 mg at bedtime) during the switch; combination therapy with lithium plus an atypical antipsychotic has been shown to be more effective than lithium monotherapy for preventing relapse. 3

Duration of Combination Therapy

  • The regimen used to stabilize acute mania should be continued for 12–24 months before considering tapering; after 6–8 weeks of stability on lithium plus aripiprazole, clinicians may evaluate the need for ongoing combination therapy versus transition to monotherapy. [1][3]
