Diagnostic Imaging Criteria for Hepatic Abscess
Characteristic Imaging Findings
- Hepatic abscesses show necrotic cavities with well-defined regions without contrast uptake in all phases, with peripheral ring enhancement during the arterial phase, rapid washout, and hypoechogenicity in the late phase, as recommended by the American Association for the Study of Liver Diseases 1, 2
- The appearance of hepatic abscesses on conventional ultrasound varies from anechoic to hyperechoic, and they may appear completely solid in some cases, with variable degrees of liquefaction 1, 2
- The European Association for the Study of the Liver suggests that ultrasound should be the first diagnostic modality used, due to its availability, moderate cost, and lack of ionizing radiation 3, 5
Diagnostic Algorithm
First-Line Diagnosis
- The American College of Gastroenterology recommends starting with abdominal ultrasound as the initial detection study 3, 5
- If ultrasound is negative but clinical suspicion remains high, consider computed tomography (CT) 6
Special Cases
- The presence of diffuse liver disease may lead to false-negative CT results, as noted by the Society of Abdominal Radiology 3, 6
- For abscesses in the hepatic dome, CT is superior to ultrasound, according to the European Society of Radiology 3, 6
Characteristics Requiring Additional Evaluation
Complex Cystic Lesions
- Atypical cystic wall or complex content requires additional evaluation with CT or magnetic resonance imaging (MRI) with contrast, as recommended by the American College of Radiology 3
- Demonstration of vascular flow within septa or solid component suggests cystic neoplasia rather than abscess, according to the European Association for the Study of the Liver 1, 2
Interventional Procedures
- Ultrasound is a validated technique for guiding interventional procedures, with a technical success rate of 96% for percutaneous or transrectal drainage, as reported by the European Society of Gastrointestinal Endoscopy 7
- CT is also useful for guiding percutaneous drainage of intra-abdominal or hepatic pyogenic abscesses, according to the Society of Interventional Radiology 7
