Molecular Profiling in Platinum-Resistant Ovarian Cancer

Introduction to Molecular Profiling

  • The National Comprehensive Cancer Network recommends against routine molecular profiling for tissue of origin identification in cancer patients, as it is a category 3 recommendation with insufficient evidence to demonstrate improved patient outcomes 1
  • The European Society for Medical Oncology recommends sequential single-agent non-platinum chemotherapy combined with bevacizumab for platinum-resistant ovarian cancer, with a primary focus on quality of life and symptom control 2

Standard Treatment Options

  • The European Society for Medical Oncology recommends sequential single-agent non-platinum chemotherapy (paclitaxel, pegylated liposomal doxorubicin, topotecan, or gemcitabine) combined with bevacizumab for platinum-resistant ovarian cancer, with a primary focus on quality of life and symptom control 2
  • The patient is already receiving gemcitabine and bevacizumab, which represents guideline-concordant therapy 2

Biomarker Testing

  • The American Society of Clinical Oncology recommends testing for folate receptor alpha (FRα) expression for potential mirvetuximab soravtansine eligibility, as it has demonstrated an objective response rate of 42.3% and median overall survival of 16.46 months in FRα-high platinum-resistant ovarian cancer patients 2
  • BRCA mutation testing and homologous recombination deficiency (HRD) testing are already standard for determining PARP inhibitor eligibility, and do not require comprehensive profiling 6, 7

Predictive Biomarkers

  • Attempts to identify predictive molecular biomarkers for bevacizumab efficacy have failed to identify any that could help decide who should receive VEGF-targeted therapies 6
  • Only clinical biomarkers (stage, debulking status, presence of ascites) appear to have predictive utility for bevacizumab benefit 6

Immunotherapy

  • Tumor mutational burden (TMB) and microsatellite instability (MSI) testing can identify candidates for immune checkpoint inhibitors, but ovarian cancers typically have low tumor mutational burden and immune checkpoint inhibitors have not shown significant benefit in unselected ovarian cancer populations 4, 5, 6

Clinical Recommendations

  • Order targeted FRα expression testing specifically for mirvetuximab soravtansine eligibility, as it has demonstrated survival benefit in platinum-resistant ovarian cancer patients 2
  • Continue current gemcitabine/bevacizumab regimen with appropriate G-CSF support for neutropenia management 8
  • Integrate palliative care early, as it significantly improves quality of life, decreases symptom intensity, and possibly improves survival in platinum-resistant disease 2
