Amiodarone Administration Guidelines

Standard Loading Protocol

  • The American College of Cardiology recommends administering IV amiodarone with a bolus dose of 150 mg over 10 minutes, followed by continuous infusion for life-threatening ventricular arrhythmias 1
  • The standard loading protocol includes a loading infusion of 150 mg over 10 minutes, followed by 1 mg/min for 6 hours and 0.5 mg/min for the remaining 18 hours, resulting in a total of approximately 1000 mg in the first 24 hours 1

Critical Administration Requirements

  • Continuous ECG monitoring is mandatory during bolus administration to detect bradycardia, heart block, QT prolongation, and hypotension, which occur in 4.9%, unknown, unknown, and 16% of IV amiodarone patients, respectively 1
  • The FDA recommends using a central venous catheter whenever possible, as peripheral vein phlebitis occurs frequently with concentrations >2 mg/mL 1

Drug Interactions Affecting Bolus Safety

  • Assess for concomitant medications that increase bradycardia risk before bolusing, including beta-blockers, calcium channel blockers, and digoxin, which create additive AV nodal suppression effects and substantially increase risk of heart block during bolus administration 1
  • Reduce digoxin dose by 50% when starting amiodarone due to increased risk of heart block 1, 2

Special Population Considerations

  • The American College of Cardiology recommends using caution with high-dose amiodarone (≥400 mg/day) in patients with severe heart failure refractory to other medications 3, 4
  • The standard bolus protocol remains appropriate for acute life-threatening arrhythmias in patients with severe heart failure, as amiodarone does not typically produce clinically significant cardiodepression in patients with left ventricular dysfunction 3, 4
