Management of Hepatic Encephalopathy in Patients with Cirrhosis and Renal Dysfunction

Introduction to L-Ornithine L-Aspartate Administration

  • The American Association for the Study of Liver Diseases recommends that L-Ornithine L-Aspartate (LOLA) should be administered with caution in patients with Stage 2-3 Acute Kidney Injury (AKI), advanced Chronic Kidney Disease (CKD), or significant Cardiovascular Disease (CVD) due to lack of safety data and contraindications to standard vasoconstrictor therapy 1, 2

Clinical Context and Risk Assessment

  • The European Association for the Study of the Liver suggests that patients with CKD stages 3-5 should be evaluated for volume overload risk before albumin administration, as it represents a relative contraindication 3
  • The American College of Cardiology recommends that terlipressin, a preferred vasoconstrictor for Hepatorenal Syndrome-AKI (HRS-AKI), is contraindicated in patients with major CVD, limiting HRS-AKI treatment options 4

LOLA Administration Protocol

  • The American Association for the Study of Liver Diseases recommends a dosage of 30 g/day as continuous intravenous infusion over 24 hours for 5 days, to be used in combination with lactulose and rifaximin for hepatic encephalopathy treatment 1, 2, 5
  • The European Association for the Study of the Liver states that oral LOLA is not recommended due to lack of efficacy 2

Treatment Algorithm

  • The American College of Gastroenterology recommends that patients with AKI should be stabilized before considering LOLA therapy, by withdrawing diuretics, beta-blockers, and nephrotoxic drugs, and administering albumin 1 g/kg (maximum 100 g/day) for volume resuscitation over 48 hours 3, 4
  • The International Society of Nephrology suggests that patients with prolonged AKI requiring renal replacement therapy (RRT) for >6 weeks should be evaluated for simultaneous liver-kidney transplant 4

Monitoring Parameters

  • The American Association for the Study of Liver Diseases recommends daily monitoring of serum creatinine, electrolytes, plasma ammonia concentrations, hepatic encephalopathy grade, and fluid balance 1, 2, 3
  • The European Society of Cardiology suggests monitoring cardiovascular status, including blood pressure, heart rate, and signs of ischemia 4
