Lithium Tapering Protocol
General Guidelines
- Tapering should be done in collaboration with the patient, allowing for adjustments based on individual response and tolerance, as recommended by the Centers for Disease Control and Prevention, based on MMWR Recommendations and Reports 1
- Regular follow-up (at least monthly) should be scheduled during the tapering process to monitor for withdrawal symptoms and adjust the taper as needed, according to the MMWR Recommendations and Reports 1
Monitoring and Adjustment
- Watch for withdrawal symptoms including anxiety, insomnia, irritability, and mood changes, as advised by the Mayo Clinic 2
- If clinically significant withdrawal symptoms occur, slow the taper rate further, as recommended by the MMWR Recommendations and Reports 1
- The taper might need to be paused and restarted when the patient is ready, especially as lower dosages are reached, according to the MMWR Recommendations and Reports 1
Discontinuation Considerations
- Abrupt discontinuation of lithium should be avoided as it can lead to withdrawal symptoms and increased risk of relapse, as warned by the Mayo Clinic 2
- Consider using adjunctive medications to manage withdrawal symptoms if they occur, as suggested by the Mayo Clinic 2
- For patients struggling with the taper, consider pausing at a stable dose before continuing the reduction, as recommended by the MMWR Recommendations and Reports 1
Tapering Strategies
- When reaching the smallest available dose, extend the interval between doses before complete discontinuation, as advised by the MMWR Recommendations and Reports 1
- When reaching lower doses that cannot be achieved with available tablet strengths, consider alternate-day dosing to achieve the equivalent average daily dose, according to the MMWR Recommendations and Reports 1
Lithium Discontinuation Guidelines
Proper Discontinuation Protocol
- A decrease of 10% of the original dose per week is a reasonable starting point for tapering, as recommended by the MMWR Recommendations and Reports 3
- The decision to discontinue lithium should be made in consultation with your prescribing physician who can evaluate your specific clinical situation, according to the American Academy of Family Physicians 4
