Management of Topical Steroid Therapy in Diabetic Patients

Indications and Usage

  • For localized disease, the British Journal of Dermatology recommends using the lowest potency steroid effective for the condition, applied to lesional skin only 1
  • For moderate to severe dermatologic conditions, very potent topical steroids may be necessary, but require close glucose monitoring, as recommended by the British Journal of Dermatology 1

Monitoring and Adjusting Diabetes Management

  • The American Diabetes Association and other guideline societies imply that monitoring blood glucose levels more frequently during topical steroid therapy is necessary, particularly with potent formulations or large surface area application 2
  • Checking glucose 2-4 hours after application, when hyperglycemic effect is maximal, is recommended, not just fasting levels, according to the American Association of Clinical Endocrinologists 4
  • Target blood glucose range should be 80-180 mg/dL during steroid therapy, as suggested by the American Diabetes Association 2
  • For patients on oral diabetes medications, continuing baseline medications and monitoring blood glucose 4 times daily during steroid therapy is advised, based on guidelines from the European Association for the Study of Diabetes 3
  • For patients already on insulin, considering increasing total daily insulin dose by 30-50% during topical steroid treatment if systemic absorption is suspected, is recommended by the American Diabetes Association 3
  • If significant hyperglycemia develops, considering temporary insulin therapy with NPH insulin 0.1-0.2 units/kg/day administered in the morning, is suggested by the American Association of Clinical Endocrinologists 2

Special Considerations

  • Elderly patients or those with renal impairment should start with lower insulin doses if insulin becomes necessary, as recommended by the International Diabetes Federation 2
