Management of Alcohol Dependence

Medication Management

  • The American Association for the Study of Liver Diseases recommends that thiamine is essential in patients with alcohol dependence to prevent Wernicke's encephalopathy, a potentially fatal complication 1, 3
  • The recommended dosing for thiamine is 100-300 mg/day for 4-12 weeks for prevention of Wernicke's encephalopathy, and 100-500 mg/day for 12-24 weeks for treatment of established Wernicke's encephalopathy 1, 3
  • Thiamine should be administered before any glucose-containing fluid, as intravenous glucose can precipitate an acute thiamine deficiency 2, 3
  • Risperidone may be used with caution as adjunctive therapy in cases of agitation or psychotic symptoms not controlled by benzodiazepines, with a suggested dosing of 0.5-5 mg every 8-12 hours orally or 2-5 mg intramuscularly 1
  • Benzodiazepines are the first-line treatment for alcohol withdrawal syndrome, with options including chlordiazepoxide, diazepam, and lorazepam, which should be used with caution and under medical supervision 1, 2

Treatment of Alcohol Withdrawal

  • The American College of Gastroenterology recommends that abstinence is the most important treatment for patients with alcoholic liver disease, and baclofen may be effective for maintaining abstinence in patients with cirrhosis 1
  • Acamprosate may be useful for maintaining abstinence after resolution of withdrawal symptoms 1

Prevention of Wernicke's Encephalopathy

  • The National Institute on Alcohol Abuse and Alcoholism recommends that thiamine should be administered before glucose-containing fluids to prevent Wernicke's encephalopathy 3
  • The intravenous route is preferred for initial treatment due to potentially poor gastrointestinal absorption in alcoholic patients 3
