Sertraline-Associated Rash Management

Clinical Evaluation and Decision Making

  • The American Academy of Family Physicians recommends examining the patient in person or requesting detailed photographs to assess rash distribution, morphology, and extent, as well as presence of mucosal involvement, blistering, or purpura, and vital signs and systemic symptoms, including lymphadenopathy or hepatosplenomegaly 2
  • For mild, localized, non-progressive rash without systemic symptoms, the American Academy of Family Physicians suggests continuing sertraline with close monitoring, prescribing symptomatic treatment, and monitoring closely for progression over 48-72 hours 2, 3
  • Discontinuation of sertraline is recommended if the rash worsens, spreads, or systemic symptoms develop, with rash typically resolving within 2 days of discontinuation with symptomatic treatment, and consideration of alternative antidepressant after rash resolution 3

Alternative Antidepressant Options

  • The American Academy of Family Physicians notes that cross-sensitivity between SSRIs has been documented, though uncommon, and recommends using extreme caution and close monitoring if switching to another SSRI, considering alternative antidepressant classes, such as bupropion or mirtazapine, if SSRI continuation is necessary 3, 4

Follow-up and Documentation

  • The American Academy of Family Physicians recommends scheduling follow-up within 48-72 hours to reassess the patient, and waiting at least 7-10 days after rash resolution before initiating alternative therapy 4
