Ceftazidime Administration Guidelines

Patient Selection Criteria

  • The American Academy of Allergy, Asthma, and Immunology recommends administering ceftazidime directly without skin testing in patients without a prior history of cephalosporin allergy, regardless of penicillin allergy history 1, 2
  • Patients with unverified penicillin allergy can receive ceftazidime without penicillin skin testing, as the cross-reactivity rate is less than 5% 1, 2
  • The cross-reactivity rate for low-similarity cephalosporins like ceftazidime is only 2.11% (95% CI: 0.98-4.46) 2
  • Even in confirmed penicillin-allergic patients, the reaction rate to ceftazidime remains very low at 0.8% (95% CI: 0.13%-4.1%) 1, 2

Special Considerations

  • Patients with a history of anaphylaxis to any cephalosporin should avoid ceftazidime entirely and select an alternative antibiotic 1
  • Patients with ceftazidime allergy should also avoid aztreonam due to shared R1 side chain cross-reactivity 5, 6
  • Skin testing to ceftazidime itself may be advisable in patients with multiple drug allergies, history of severe/recurrent reactions, or significant patient anxiety 1, 2, 3, 5, 7

Skin Testing Protocol

  • If skin testing is deemed necessary, use the following concentrations:

Critical Pitfalls to Avoid

  • Do not assume 10% cross-reactivity between penicillins and cephalosporins, as modern evidence shows less than 5% for unverified and approximately 2% for confirmed allergies 1, 2
  • Do not perform penicillin skin testing before ceftazidime administration unless the patient has a history of anaphylaxis to cephalosporins and you need to determine if penicillins are safe alternatives 1, 3
  • Do not confuse ceftazidime with cephalexin/cefaclor (aminocephalosporins), which have higher cross-reactivity rates with aminopenicillins due to shared R1 side chains 2, 4
