Metabolic Monitoring for Patients Taking Aripiprazole

Initial Baseline Testing and Follow-up Monitoring

  • Fasting blood glucose measurement is necessary as aripiprazole can affect glucose metabolism, with baseline values needed for comparison during treatment 1
  • Fasting blood glucose should be checked within the first 1-2 months after starting aripiprazole and then every 3-4 months thereafter, especially for patients with risk factors for diabetes 1
  • Hemoglobin A1c testing may be considered as an alternative or supplement to fasting glucose measurements 1
  • Patients with pre-existing diabetes or metabolic syndrome require more vigilant monitoring, potentially monthly for the first 3 months 1

Special Considerations for Liver Function and Hematological Monitoring

  • More frequent monitoring of liver enzymes (ALT, AST) may be necessary for patients with pre-existing liver disease or who are taking other hepatotoxic medications 2
  • CBC with differential should be checked within 1-2 months of starting therapy and then every 3-4 months to monitor for potential hematological adverse effects 2
  • Patients on multiple psychotropic medications should follow the more frequent monitoring schedule if there are different recommendations for each medication 2
