Management of Hypoxemia in Cancer Patients

Immediate Assessment and Isolation

  • The European Society for Medical Oncology recommends testing for SARS-CoV-2 immediately and isolating the patient until results are available, as cancer patients with fever and hypoxemia are at high risk of mortality if infected 1, 2
  • The American College of Physicians suggests segregating/isolating the patient immediately pending COVID-19 swab results, and healthcare workers must use full personal protective equipment (PPE) including eye protection, gown, and surgical mask or FFP respirator 1, 2

Oxygen Therapy Management

  • The American Thoracic Society recommends starting supplemental oxygen immediately targeting SpO₂ 94-98% in cancer patients with hypoxemia 3
  • The European Respiratory Society suggests monitoring oxygen saturation at least every 4 hours during acute illness, positioning the patient upright to optimize ventilation unless contraindicated, and measuring respiratory rate and heart rate carefully 3

Airway Assessment Priority

  • The American Academy of Otolaryngology recommends evaluating immediately for potential airway compromise from the soft palate malignancy, as soft palate tumors can cause upper airway obstruction 4, 5
  • The British Journal of Anaesthesia suggests discussing urgently with an intensivist for potential ICU transfer and considering fibreoptic bronchoscopy if respiratory symptoms worsen or hypoxemia persists despite oxygen 4

Infectious Workup

  • The Infectious Diseases Society of America recommends obtaining blood cultures given fever in immunocompromised cancer patients, and considering atypical infections including listeriosis if dietary history includes soft cheeses 2, 6
  • The American College of Radiology suggests obtaining a chest radiograph only if respiratory signs or symptoms develop, and considering a complete blood count to assess for neutropenia 6

Monitoring for Deterioration

  • The American College of Chest Physicians recommends monitoring for worsening hypoxemia requiring escalation to high-flow oxygen or non-invasive ventilation, development of respiratory distress, signs of airway obstruction, and hemodynamic instability 3

Treatment of Underlying Cause

  • The National Comprehensive Cancer Network recommends following institutional protocols for COVID-19 management in cancer patients if COVID-19 positive, and targeted antibiotic therapy if bacterial infection identified 1, 3
  • The European Society for Medical Oncology suggests urgent oncology consultation for assessment of disease progression and potential palliative interventions if tumor-related 2
