Management of Severe Mixed Dyslipidemia
Risk Stratification and Treatment
- The American Heart Association guidelines state that LDL-C ≥190 mg/dL warrants pharmacotherapy regardless of other risk factors or cardiovascular disease, with a goal of reducing LDL-C to <100 mg/dL 1, 3
- Patients with severe mixed dyslipidemia should have their LDL-C, HDL-C, and triglycerides monitored, with goals of LDL-C <100 mg/dL, HDL-C >50 mg/dL in women, and triglycerides <150 mg/dL 1, 2
First-Line Pharmacotherapy
- The American College of Cardiology recommends starting moderate-to-high intensity statin therapy immediately, with a goal of achieving at least 30-40% LDL-C reduction from baseline 1, 2
- Statin therapy should be initiated with a dose of atorvastatin 20-40 mg daily or rosuvastatin 10-20 mg daily, with monitoring of liver enzymes and creatine kinase at baseline and as clinically indicated 1, 2
Lifestyle Modifications
- The American Heart Association recommends reducing saturated fat to <7% of total calories, limiting cholesterol intake to <200 mg/day, and eliminating trans-fatty acids to <1% of energy 1, 3
- Patients should aim to achieve/maintain a BMI of 18.5-24.9 kg/m² and waist circumference <35 inches, and engage in at least 30 minutes of moderate-intensity aerobic activity most days of the week 1
Secondary Pharmacotherapy
- For patients with elevated triglycerides, omega-3 fatty acids (2-4 grams EPA+DHA daily) or fibrate therapy (fenofibrate) can be considered after LDL-C goal is reached 1, 2, 3
- For patients with low HDL-C, niacin or fibrate therapy can be useful when HDL-C remains low after LDL-C goal is achieved, with a Class IIa recommendation from the American Heart Association 1, 2, 3
Monitoring and Additional Risk Reduction
- Lipid panels should be rechecked in 4-12 weeks after statin initiation, with assessment of LDL-C response and adjustment of statin dose if needed 4
- Blood pressure goal should be <120/80 mmHg, with screening for diabetes mellitus and consideration of aspirin 81 mg daily if 10-year cardiovascular risk >10% and blood pressure is controlled 1, 2, 3, 5
