Minoxidil Non‑Response: Genetic Factors and Evidence‑Based Combination Strategies

Laboratory Findings vs Follicular Enzyme Activity

  • Normal serum levels of TSH, vitamin D, zinc, and ferritin do not predict clinical response to topical minoxidil because efficacy depends on follicular sulfotransferase activity, which varies genetically among individuals. 1

Combination Therapy with Platelet‑Rich Plasma (PRP)

  • Adding monthly PRP injections to twice‑daily 5 % topical minoxidil yields significantly greater hair regrowth than either modality alone, demonstrating superior efficacy in patients who fail minoxidil monotherapy. 3
  • The recommended regimen for responders is topical minoxidil 5 % applied twice daily combined with PRP injections for at least three monthly sessions, followed by maintenance injections every six months. 4
  • This combination produces approximately a 1.7‑fold increase in hair‑density and a 14‑fold increase in hair‑diameter compared with minoxidil alone. 5

Oral Minoxidil and Cardiovascular Safety

  • Low‑dose oral minoxidil (0.5–5 mg daily) may be considered for patients who do not respond to topical therapy, but it is an off‑label use that requires mandatory cardiovascular screening before initiation. 4
  • Oral minoxidil is contraindicated in individuals with pre‑existing cardiovascular disease due to the risk of systemic side‑effects. 4
