Borderline Personality Disorder Diagnosis and Treatment

Diagnostic Criteria

  • The American Academy of Child and Adolescent Psychiatry notes that a history of rapid mood shifts is strongly associated with Borderline Personality Disorder (BPD), and diagnosing such patients is complex due to consideration of multiple diagnoses including major depressive disorder, bipolar disorder, or BPD 1, 2
  • Unstable interpersonal relationships, such as fear of abandonment and difficulty maintaining close relationships, are characteristic of BPD 1, 2
  • Rapid mood shifts, emotional instability, and brief periods of depression, anxiety, and rage are common in BPD patients 1, 2
  • Identity disturbance, marked by oscillating self-concept, is a key feature of BPD 2
  • Impulsivity, including calling people back immediately after hanging up and obsessive checking behaviors, is a diagnostic criterion for BPD 1, 2
  • Affective instability, characterized by marked reactivity to small triggers, is a hallmark of BPD 1, 2

Treatment Approach

  • The American Academy of Child and Adolescent Psychiatry recommends Dialectical Behavior Therapy (DBT) as the primary treatment for BPD, with effect sizes of 0.50-0.65 for core BPD symptom severity 1, 2
  • Cognitive Behavioral Therapy (CBT) should be integrated to address comorbid anxiety symptoms, as it is the most effective psychotherapeutic approach for Generalized Anxiety Disorder (GAD) and panic disorder 3, 4

Critical Safety Assessment

  • Immediate suicide risk evaluation is mandatory in BPD patients, given the high-risk features such as emotional instability, impulsivity, and interpersonal relationship problems 1, 2
  • The American Academy of Child and Adolescent Psychiatry emphasizes that patients who are irritable, agitated, or voice persistent wishes to die pose greater short-term suicide risk and require immediate psychiatric referral 1, 2
