Nitrofurantoin Use in Chronic Kidney Disease: Evidence‑Based Recommendations

Contraindications

  • Absolute contraindication: In patients with an estimated glomerular filtration rate (eGFR) < 30 mL/min/1.73 m², nitrofurantoin should not be prescribed because urinary drug concentrations become subtherapeutic and the risk of adverse effects rises. (moderate‑level evidence) 1
  • Expert consensus: Geriatric clinical pharmacists advise avoiding nitrofurantoin when creatinine clearance falls below 30 mL/min in older adults. (expert consensus) 1

Renal Function Assessment

  • eGFR calculation required: Before initiating nitrofurantoin, clinicians must calculate eGFR rather than rely solely on serum creatinine, as creatinine can appear normal despite significant renal impairment in older adults or those with low muscle mass. (expert recommendation) 1
  • Serum creatinine is insufficient: Normal serum creatinine does not guarantee normal kidney function in older adults or patients with reduced muscle mass; eGFR should always be used to assess renal status. (expert recommendation) 1

Prescribing Guidance

  • Do not use nitrofurantoin when eGFR < 30 mL/min/1.73 m²: Choose an alternative antibiotic for urinary tract infection treatment in this renal function range. (guideline recommendation) 1
