Urgent Gastroenterology Referral and Full Colonoscopy with Biopsies

Introduction to Chronic Diarrhea Diagnosis

  • The American Gastroenterological Association recommends that patients with chronic diarrhea and alarm features such as nocturnal diarrhea undergo urgent gastroenterology referral within 2-4 weeks for full colonoscopy with biopsies from both right and left colon, even if the mucosa appears normal 1, 2
  • Nocturnal diarrhea strongly indicates organic pathology rather than functional disorders and warrants expedited evaluation, according to the American College of Gastroenterology 3, 4, 1

Diagnostic Testing

  • The European Society for Clinical Nutrition and Metabolism recommends complete blood count to assess for anemia from iron, B12, or folate deficiency 1, 2
  • The American Gastroenterological Association suggests C-reactive protein or erythrocyte sedimentation rate for inflammatory markers 1, 2
  • The National Institute for Health and Care Excellence recommends anti-tissue transglutaminase IgA with total IgA for celiac disease screening, given steatorrhea pattern 1, 2
  • The American College of Gastroenterology recommends comprehensive metabolic panel to evaluate electrolyte disturbances from chronic diarrhea 1, 2
  • The Endocrine Society suggests thyroid function tests to exclude hyperthyroidism 1, 2

Priority Differential Diagnoses

  • The American Gastroenterological Association lists celiac disease as a priority differential diagnosis, given steatorrhea pattern, with a recommendation for strict lifelong gluten-free diet 5, 3, 2
  • The European Society for Clinical Nutrition and Metabolism recommends considering chronic pancreatitis or pancreatic carcinoma as a cause of pancreatic exocrine insufficiency 5, 3
  • The American College of Gastroenterology suggests small bowel bacterial overgrowth as a potential cause of malabsorption 5, 3
  • The Infectious Diseases Society of America recommends considering giardiasis as a classic infectious malabsorptive cause 5, 3

Critical Pitfalls to Avoid

  • The American Gastroenterological Association advises against diagnosing irritable bowel syndrome in patients with nocturnal diarrhea, as it is an exclusion criterion for functional disorders 1, 2
  • The American College of Gastroenterology recommends against skipping colonic biopsies, even with normal-appearing mucosa, as microscopic colitis requires histologic diagnosis 1, 2
  • The American Society for Gastrointestinal Endoscopy advises against performing flexible sigmoidoscopy alone, as full colonoscopy is required to evaluate the entire colon 1, 2

Treatment

  • The American Gastroenterological Association recommends strict lifelong gluten-free diet for celiac disease 1
  • The American College of Gastroenterology suggests budesonide for microscopic colitis 1
  • The European Society for Clinical Nutrition and Metabolism recommends cholestyramine or bile acid sequestrants for bile acid diarrhea 5, 1
  • The American Gastroenterological Association advises against using empiric loperamide until organic causes are excluded, as symptomatic treatment will mask the underlying diagnosis and delay appropriate therapy 4, 1, 2
