Postpartum Chorioamnionitis Treatment

Antibiotic Regimen

  • The American College of Obstetricians and Gynecologists recommends adding clindamycin 900 mg IV at umbilical cord clamping if delivery is by cesarean section, providing additional anaerobic coverage for the surgical procedure 1

Penicillin Allergy Considerations

  • For patients with non-severe penicillin allergy, cefazolin is the preferred alternative 1
  • For patients with severe penicillin allergy, clindamycin or vancomycin should replace ampicillin, and gentamicin should be maintained for gram-negative coverage 1

Neonatal Implications

  • The American Academy of Pediatrics recommends that all newborns born to mothers with chorioamnionitis require limited evaluation, including blood culture and complete blood count with differential and platelet count 1, 2
  • Neonates should receive empiric antibiotics pending culture results, such as ampicillin plus gentamicin or cefotaxime, for gram-negative and GBS coverage 1, 3

Critical Pitfalls

  • The Society for Maternal-Fetal Medicine emphasizes the importance of not forgetting clindamycin at cesarean delivery, as anaerobic coverage is critical for surgical site infection prevention 1
