Weaning Continuous 3% Hypertonic Saline

Monitoring Requirements During Weaning

  • The American Society of Anesthesiologists recommends measuring serum sodium within 6 hours of any dose adjustment and not proceeding with further weaning until confirming sodium concentration is <155 mmol/L 1, 2
  • Continue ICP monitoring during the weaning process to detect rebound intracranial hypertension 2

Stepwise Weaning Protocol

  • The American College of Critical Care Medicine suggests verifying the clinical indication for discontinuation has resolved before initiating weaning, and confirming current serum sodium is in the 145-155 mmol/L range 1, 2
  • Reduce infusion rate by 25-50% every 12-24 hours if serum sodium remains stable between 145-150 mmol/L, with a mean treatment duration of 7.6 days 1

Critical Safety Parameters

  • Stop weaning and resume previous infusion rate if serum sodium drops >5 mmol/L in 6 hours or if clinical deterioration occurs 1
  • Hold further weaning if sodium exceeds 155 mmol/L at any point 1, 2

Common Pitfalls to Avoid

  • Avoid measuring sodium less frequently than every 6 hours during active weaning, as rapid changes can occur 1, 2

Post-Discontinuation Monitoring

  • Measure serum sodium 6 hours after complete discontinuation 1, 2
  • Maintain ICP monitoring for at least 24 hours post-discontinuation in neurocritical patients 2

Administration of 3% Saline Boluses for Raised Intracranial Pressure

Dosing Guidelines for 3% Saline

  • For acute management of elevated intracranial pressure (ICP), the standard dose is 5 mL/kg of 3% hypertonic saline administered over 15-20 minutes per bolus, with maximum effect observed after 10-15 minutes and lasting for 2-4 hours, which informs the timing between boluses 3
  • In severe cases of hyponatremia with neurological symptoms, up to three 100 mL boluses of 3% saline spaced at 10-minute intervals are initially recommended 4

Monitoring Requirements When Exceeding 3 Boluses

  • Serum sodium levels must be measured within 6 hours of bolus administration, and additional boluses should not be administered until serum sodium concentration is confirmed to be <155 mmol/L 3
  • Continuous monitoring of ICP is essential during therapy, especially when multiple boluses are required 3

Special Considerations

  • For patients with cerebral salt wasting, fluid restriction is contraindicated as it may increase the risk of cerebral infarction 5
  • No evidence of osmotic demyelination syndrome has been reported with proper monitoring despite multiple boluses 3
