Tetanus Vaccination in Immunocompromised Patients

Introduction to Tetanus-Containing Vaccines

  • The Advisory Committee on Immunization Practices (ACIP) recommends that tetanus-containing vaccines (Td or Tdap) can be administered to patients undergoing chemotherapy, as immunosuppression is not a contraindication to these inactivated vaccines 1, 2, 3

Key Guideline Recommendations

  • The ACIP clearly states that immunosuppression, including in patients receiving chemotherapy, is a condition under which Td or Tdap may be administered if otherwise indicated 1, 2, 3
  • The Centers for Disease Control and Prevention (CDC) advises that vaccination should not be withheld from immunosuppressed patients, as some protection is better than none, particularly given the high morbidity and mortality of tetanus infection 1, 2

Clinical Algorithm for Chemotherapy Patients

  • The ACIP recommends administering Tdap if the patient has never received it, regardless of chemotherapy status 1, 2, 4
  • The CDC advises giving Td boosters every 10 years for ongoing tetanus/diphtheria protection 2, 4
  • The ACIP states that vaccination should not be deferred due to chemotherapy alone 1, 2, 3

Wound Management

  • The CDC recommends following standard tetanus prophylaxis guidelines based on wound type and vaccination history 2, 4
  • For clean, minor wounds, the ACIP advises giving tetanus vaccine if more than 10 years have passed since the last dose 4
  • For contaminated or tetanus-prone wounds, the CDC recommends giving tetanus vaccine if more than 5 years have passed since the last dose 4
  • The ACIP prefers Tdap over Td if the patient has not previously received Tdap 2, 4

Contraindications and Precautions

  • The ACIP lists a history of anaphylaxis to vaccine components as a contraindication to vaccination, and recommends referral to an allergist for possible tetanus toxoid desensitization 1, 2, 4
  • The CDC notes that encephalopathy within 7 days of previous pertussis-containing vaccine is a contraindication for Tdap, but not for Td 1, 2, 4
  • The ACIP considers Guillain-Barré syndrome within 6 weeks of previous tetanus-containing vaccine a precaution, rather than an absolute contraindication, and recommends evaluating the risks and benefits of vaccination 1, 3, 4

Common Pitfalls to Avoid

  • The ACIP warns against withholding tetanus vaccination from immunosuppressed patients, as this leaves them vulnerable to a preventable disease with high mortality 1, 2, 3
  • The CDC advises against giving tetanus boosters more frequently than every 10 years for routine immunization, as this can cause adverse reactions 1, 3, 4
  • The ACIP recommends not deferring vaccination for moderate or severe acute illness, but rather waiting until the acute illness resolves, as chemotherapy itself is not an acute illness requiring deferral 1, 3, 4
