Treatment Options for Metastatic Castration-Resistant Prostate Cancer (mCRPC)

Clinical Context and Treatment Sequencing

  • The American Urological Association (AUA) guidelines describe a patient with symptomatic mCRPC, good performance status, and prior docetaxel chemotherapy, now with additional progression through multiple subsequent lines including abiraterone and Lu-177-PSMA, similar to Index Patient 5 2, 3

Primary Treatment Recommendation: Cabazitaxel

  • Cabazitaxel is FDA-approved for mCRPC patients who have progressed after docetaxel-based chemotherapy, representing the standard of care in this setting, with proven overall survival benefit in post-docetaxel mCRPC 1
  • The drug works independently of PSMA expression, addressing heterogeneous disease, and is effective against both bone and visceral metastases, without requiring specific biomarker expression for efficacy 1

Alternative Considerations

  • Enzalutamide could be considered if not previously used, though prior abiraterone exposure and CHEK2 mutation status suggest potential cross-resistance, with less robust evidence for PARP inhibitor response compared to BRCA mutations 1, 4, 2

Monitoring During Treatment

  • PSA should not be the sole endpoint, with clinical and radiographic progression being more meaningful than PSA changes alone, and regular assessment of pain control and quality of life measures 6, 1, 5

Supportive Care Considerations

  • Bone health management remains critical, with continuation of bone-targeted therapy (denosumab or zoledronic acid), optimization of pain management, and consideration of palliative radiation for symptomatic bone lesions 5

Future Treatment Planning

  • After cabazitaxel progression, consider clinical trial enrollment, comprehensive genomic profiling to identify targetable alterations beyond CHEK2, and palliative care integration for symptom management and quality of life optimization 1, 5
