Administration of Lovenox in the Thighs

  • The American Academy of Orthopaedic Surgeons recommends the abdomen as the most commonly used and traditionally recommended site for subcutaneous Lovenox injections 1
  • The anterolateral and posterolateral aspects of the thigh are appropriate alternative injection sites for subcutaneous administration of Lovenox 2

Proper Administration Technique

  • Lovenox should be administered via deep subcutaneous injection (not intramuscular) 2
  • When using the thigh as an injection site, proper technique includes selecting the anterolateral or posterolateral aspect of the thigh, pinching a fold of skin between thumb and forefinger, inserting the entire needle length at a 90-degree angle, releasing the skin fold and injecting the medication slowly, and not rubbing the injection site after administration to prevent bruising 2

Clinical Advantages of Enoxaparin

  • Enoxaparin has a more predictable anticoagulation effect compared to unfractionated heparin 2
  • Enoxaparin has a lower risk of heparin-induced thrombocytopenia compared to unfractionated heparin 2
  • Enoxaparin has a lower risk of osteopenia with prolonged use compared to unfractionated heparin 2

Dosing Considerations

  • The standard prophylactic dose is 40 mg subcutaneously once daily for most patients 2
  • For patients with class III obesity (BMI ≥40 kg/m²), consider intermediate dosing of 40 mg subcutaneously every 12 hours or weight-based dosing 2
  • The duration of prophylaxis should be throughout hospitalization or until fully ambulatory, with a minimum of 7-10 days for surgical patients 2
