Treatment of Sleep Deprivation

First-Line: Behavioral Interventions

  • The American Academy of Sleep Medicine recommends Cognitive Behavioral Therapy for Insomnia (CBT-I) as the first-line treatment for sleep deprivation and chronic insomnia, with sleep restriction therapy and stimulus control as core components that directly address the underlying sleep architecture problems 1, 2
  • CBT-I should be implemented before considering pharmacological options, and it is the standard treatment recommended by the American Academy of Sleep Medicine 1, 2
  • Stimulus control therapy, a component of CBT-I, involves going to bed only when sleepy, leaving bed if unable to sleep within 20 minutes, and maintaining a regular sleep-wake schedule, with the goal of associating the bed with sleep 1, 2
  • Sleep restriction therapy, another component of CBT-I, involves calculating mean total sleep time from 1-2 week sleep logs, setting time in bed to match actual sleep time, and adjusting weekly based on sleep efficiency, with the goal of improving sleep quality 1, 2
  • Relaxation training, such as progressive muscle relaxation, can be used to reduce somatic and cognitive arousal and improve sleep 1
  • Cognitive therapy can be used to address maladaptive beliefs about sleep, such as "I can't sleep without medication" or "My life will be ruined if I can't sleep" 1

Sleep Hygiene Education

  • The American Academy of Sleep Medicine recommends incorporating sleep hygiene education into CBT-I, including maintaining a regular sleep-wake schedule, avoiding caffeine after noon, limiting alcohol, keeping the bedroom dark and cool, avoiding heavy meals near bedtime, and engaging in regular morning or afternoon exercise 1, 3, 4

Environmental Optimization for ICU Patients

  • The American Thoracic Society recommends optimizing the environment for critically ill patients with sleep deprivation, including controlling light and noise, clustering care activities, and protecting sleep cycles during 2-4 AM or 12-5 AM periods 6, 7

Second-Line: Pharmacological Treatment

  • The American Academy of Sleep Medicine recommends considering pharmacological options only after 4-8 weeks of unsuccessful behavioral interventions 2
  • Eszopiclone can be effective for sleep maintenance insomnia, but should be started at lower doses in elderly patients and monitored for next-day psychomotor impairment 2
  • Temazepam can be used for sleep maintenance, but should be avoided for long-term use due to dependence, tolerance, and cognitive impairment risks, particularly in elderly patients 2

Special Populations

  • The American Geriatrics Society recommends using extreme caution with benzodiazepines in elderly patients due to high fall risk, and tapering and discontinuing if currently prescribed 5
  • The American Academy of Sleep Medicine recommends reducing sedative medication doses by at least 50% in renal impairment 5
  • The American Academy of Sleep Medicine recommends screening for sleep apnea and restless legs syndrome in elderly patients, as these conditions are common in this population 5
  • The Society of Critical Care Medicine recommends prioritizing nonpharmacologic interventions for ICU patients, including optimizing the environment, controlling light/noise, clustering care activities, and protecting sleep during 2-4 AM period 6, 7
  • The National Comprehensive Cancer Network recommends physical activity, such as yoga or exercise, to improve sleep quality and reduce the need for sleep medications in cancer survivors 4
  • The National Comprehensive Cancer Network recommends psychosocial interventions, such as CBT or mindfulness meditation, as more effective than sleep hygiene education alone for cancer survivors 4
